  • KSBs targeted this period

  • OTJ hours logged vs expected
  • OTJ pace flag (on track / at risk)

SMART targets

  • Previous targets: outcome and evidence
  • New targets: specific, measurable, time-bound
  • KSBs addressed by each target
  • Target owner (learner, employer, tutor)

Employer and sign-off

  • Employer input and concerns noted
  • Safeguarding and welfare check field
  • Learner signature (or e-sign confirmation)

Recommended review cadence

  1. Schedule reviews at 8–12 week intervals from programme start — not less frequently than every 12 weeks.
  2. Involve the employer in every review, not just the learner and tutor.
  3. Set 2–4 SMART targets per review; carry forward any unmet targets with updated timescales.
  4. Flag OTJ pace concerns at the review — do not wait for the next review cycle to intervene.
  5. Store signed records for the full programme duration plus at least two years post-gateway.

What Ofsted inspectors look for in review records

  • Evidence that the employer was genuinely involved — not just a signature at the end.
  • SMART targets that reference specific KSBs, not generic development goals.
  • Continuity between reviews — previous targets reviewed and outcomes documented.
  • OTJ hours tracked with a running total, not just a note that learning has occurred.
  • Welfare and safeguarding questions asked and answered at every review.
