How to use this template: Work through each tab in order. Score each vendor 1–5 for each criterion (1 = does not meet, 5 = exceeds requirement). Starred rows (★) are must-have criteria — a score of 1–2 on a starred item should disqualify the vendor regardless of total score. The summary tab shows weighted totals.
